PATIENT INFORMATION

To help familiarize you with our office, we have listed some of our office policies below. If you do not find what you are looking for here, please feel free to Contact Us.

APPOINTMENTS

We see patients by appointment. Please give our office at least 24 hours notice when you find that you need to cancel an appointment.

ONLINE REGISTRATION, MEDICAL HISTORY FORMS AND AUTHORIZATIONS

Prior to your appointment please register in our secure online patient portal, complete the online medical history forms and e-sign authorization forms. Alternatively, if you prefer or do not have internet access, you may also fill out paper forms at the office but please make sure to arrive a least thirty minutes prior to your appointment to fill out these forms.

Front Desk

FINANCIAL POLICY

All co-payments, deductibles and non-covered services are required to be paid at the time of service. For your convenience, we do accept checks, debit and credit cards. We are no longer accepting cash.

PRESCRIPTION REFILLS

When requesting refills, please be prepared to provide us with the name of the medication, the dosage and the phone number for your pharmacy. Please allow us until the end of our business day to get refills processed.


As a general rule, we do not process refill requests after hours or on the weekends unless it is an emergency.

INSURANCE

We accept most major health insurance plans. For a complete listing of accepted health plans, click here.


If you do not have health insurance, we will work with you on an affordable payment plan for medical services provided in our office.
